CLS Heroes: Chichester Community Social Work Team   and the Communities Team

These teams working in partnership have provided excellent support for many people and one example, where they acted as ‘a team around’ the person, is used to demonstrate this.  Supported by community services to physically move house and decorate to make it more homely, this person was also supported to equip their home through charity grants.

Their emotional wellbeing is generally improving in their new flat and, other than frequently accessing professional support, they have no other formal services in place.  This is a great example of CLS in action.
